I confirm my pet is current on core vaccinations, free of communicable illness, and safe to transport. I release PetRidez and its providers from claims arising from pre-existing conditions, undisclosed behavior, or my failure to disclose risks. I authorize emergency veterinary care up to $500 if I cannot be reached in transit.
